What happened
On May 29, 2014, a CH-7 Kompress helicopter was conducting a flight from Heringsdorf to Warsaw/Babice. While operating at an altitude of 600 meters, the aircraft entered the boundaries of the temporary segregated area TSA 09C. This specific airspace zone was active at the time and reserved for state aviation operations.
The investigation
The PKBWL investigation focused on the circumstances leading to the unauthorized entry into the restricted airspace. The inquiry examined the flight path, the altitude of the aircraft, and the crew's awareness of the active airspace restrictions along the planned route.
Findings
- The investigation determined that the crew failed to maintain separation from the boundaries of the temporary airspace.
- The primary cause of the incident was the inadequate preparation of the crew regarding information about flexible airspace elements active along the intended flight path.
