What happened
On June 15, 2017, at approximately 08:30 LMT, a SAAB SF-340A operated by SkyTaxi sp. z o.o. was preparing for departure at Wrocław-Strachowice Airport (EPWR). After the engines were started, a mechanic noticed objects falling from the aircraft's left horizontal stabilizer. In response, the engines were immediately shut down to allow for a thorough inspection.
The investigation
An investigation conducted by the operator revealed that various tools and materials had fallen from the aircraft. Upon inspecting the upper surface of the left horizontal stabilizer and the vertical fin, investigators found additional tools and debris. These items were identified as materials left behind following maintenance work performed on the dorsal fin the previous day. Following the removal of the debris, the vertical fin was verified, and the control surface deflections were checked to ensure proper functionality. The aircraft was subsequently cleared for flight, and no further irregularities were noted during subsequent operations.
Findings
- The primary cause of the incident was human error, specifically the inattention of a maintenance worker during previous servicing tasks.
- Maintenance work performed on the dorsal fin the day prior to the incident left unsecured tools and materials on the aircraft structure.
