What happened
On July 22, 2018, a tandem skydive took place near the Przylep aerodrome (EPZP). The jump involved a skydiver and a passenger exiting an aircraft at an altitude of approximately 3,300 meters. During the descent, at an altitude between 1,200 and 1,300 meters, the Sigma 370 parachute experienced an asymmetric deployment from the container.
As the canopy emerged, the deployment process was uneven. This asymmetry likely caused several lines to be pulled through the inflating canopy, resulting in a significant canopy deformation, often referred to as a "cauliflower" shape. This malfunction led to rapid spinning of the tandem pair and an increased rate of descent. To manage the emergency, the tandem pilot executed emergency procedures by cutting away the main canopy and deploying the reserve parachute. The landing was completed on the airfield surface without further incident or injuries.
The investigation
The investigation, conducted by the operator, examined the deployment sequence and the physical state of the equipment. The inquiry focused on the mechanics of the container closure and the interaction between the lines and the deployment bag.
