Incident Overview
During the takeoff roll, the right engine failed. The pilot aborted the takeoff and swerved the aircraft to avoid a blast fence at the end of the runway. The left main landing gear failed, and the aircraft subsequently collided with the fence.
Maintenance History
Maintenance records revealed a total of 12 discrepancies pertaining to fuel flow on the right engine, with the first noted on July 14, 1981.
Post-Accident Engine Testing
An engine run was conducted after the accident. When takeoff power was applied and the boost pump was turned on, the engine quit. The right engine-driven fuel pump, identified as Lear RG 17980J, was then bench tested. The fuel pump cover did not appear to be properly safety-wired. During pressure testing, fuel leaked from the vent hole in the cap. When the cover was removed, the diaphragm retaining nut fell out onto the test bench.
