Accident
On December 13, 1994, while approaching Raleigh–Durham International Airport, the captain decided to conduct a go-around after believing that one of the engines had failed. The captain had been written up for piloting deficiencies at his previous employer, a fact that Flagship had not checked before hiring him. During the go-around, the captain failed to follow any of the procedures for a single-engine go-around. The aircraft stalled and crashed into a forest southwest of the airport.
Casualties
Both crew members and 13 passengers died; the 5 surviving passengers suffered serious injuries.
