Accident
On 13 June 1996, Garuda Indonesia Flight 865, a scheduled international flight from Fukuoka, Japan, to Jakarta, Indonesia via Bali, Indonesia, crashed during takeoff from runway 16 at Fukuoka Airport. The aircraft, a McDonnell Douglas DC-10-30 registered PK-GIE, carried 275 occupants. The crew comprised Captain Ronald Longdong (38), First Officer Yudhia Putra (31), and Flight Engineer Dwi Prayitno (34).
After being cleared for takeoff, the crew attempted to abort the takeoff following failure of the number 3 (right) engine. The abort occurred at speeds nearing V2, after nose rotation. Attempts to stop the aircraft on the runway using brakes, ground spoilers, and thrust reversers were unsuccessful. The aircraft exited the runway and airport premises. The captain stated that he feared hitting buildings or objects if he did not abort.
The aircraft slid through a ditch, a fence, and a road before coming to rest approximately 620 meters beyond the runway threshold. During the slide, the landing gear broke off, both wing-mounted engines were torn from the wings, and the fuselage broke in two places: near the wing root trailing edge and about 10.4 meters aft of that point. A fire destroyed the areas between the hull fractures and other aircraft parts.
Three bodies were later discovered in the rear of the wreckage. Two passengers, in seats 34K and 35K, died from violent impact. A third passenger, seated in 35J, was rendered unconscious by the impact and could not escape the fire.
Aircraft
The aircraft, a McDonnell Douglas DC-10-30 with registration PK-GIE, was manufactured in 1979 with manufacturer serial number 46685/284. It was powered by three General Electric CF6-50C2 turbofan engines. The failure of the number 3 engine was attributed to turbine blades that had been in service for 6,182 cycles, exceeding General Electric's recommended discard limit of 6,000 cycles.
Probable Cause
The final report concluded that pilot error and the failure of the maintenance and flight operation sections of the airline in properly coordinating matters resulted in the accident.
