Aircraft Crash Near Departure Airstrip

Casualties unknown • Willow Beach, AZ, US

A search operation located an aircraft three miles southeast of its departure point following a failed return from a local flight.

What happened

After the pilot failed to return from a local flight, a search was initiated. The aircraft was subsequently located approximately 3 miles southeast of the departure airstrip. Upon discovery, it was noted that the aircraft had collided with near-level desert terrain at a 45-degree angle, showing no signs of rotation.

The investigation

Investigators examined the engine and accessories, finding them to be free of discrepancies. Evidence of engine operation was present, as one propeller blade showed signs of rotation and a tachometer needle imprint indicated approximately 975 rpm.

An unquantified amount of fuel remained in the left wing tank. The aircraft's fuel system was operating on automobile gasoline, which was an approved fuel source for this aircraft. However, investigators found internal fuel hose deterioration that was obstructing a fuel sight gauge vent line and coupling hoses. Additionally, minor contamination and a rust-colored water line were found in the fuel gascolator bowl.

Regarding the cabin configuration, the aircraft was being operated without the front seat. Furthermore, both rear seat belt attachment points, consisting of .090 steel wire around 4130 steel tubing, had separated. No shoulder harness was installed in the aircraft.
