Incident Overview
Shortly after takeoff, the pilot of the aircraft heard a loud bang and experienced a strong engine vibration. The pilot then attempted to return to the runway, but the aircraft crashed during the maneuver.
Post-Crash Findings
Examination of the engine after the accident revealed that the number two cylinder was cracked between the two spark plug portholes.
Operator's Account
The owner/operator of the aircraft reported that cracked cylinders are typical on these engines, usually due to the pilot overboosting the engine on takeoff. The owner stated that the pilot involved in this accident had cracked a cylinder on the accident aircraft on five previous occasions, as well as on other aircraft. The owner further stated that he trains all his pilots in emergency procedures.

The pilot of the aircraft did not respond to requests for the accident report and no longer works for the operator.
