What happened
On March 10, 2015, a Cessna 208B, registration N12155, was performing an ILS runway 24 approach at Muskegon, Michigan, under instrument meteorological conditions (IMC). The pilot conducted an autopilot-coupled approach to minimums. Upon reaching the decision height, the pilot disconnected the autopilot and exited IMC.
At that moment, the approach lights became visible, and the pilot realized the aircraft was positioned to the right of the runway. While attempting to maneuver the aircraft back toward the centerline, the airplane struck components of the approach light system. There were no injuries resulting from the accident. Post-accident inspection showed the aircraft's horizontal stabilizer sustained substantial damage.
The investigation
Investigators found that a permanent Notice to Airmen (NOTAM) was active for the ILS runway 24 approach. This NOTAM prohibited using a coupled autopilot approach from 1,150 feet mean sea level down to the surface. The NOTAM had been in effect since February 21, 2014. During interviews, the pilot stated he was unaware of this restriction, and the approach chart being used did not include a note regarding this specific limitation.
Weather at the time of the event included a 250-degree wind at 5 knots, 0.5 statute mile visibility, and a 200-foot overcast ceiling. The pilot reported no mechanical failures or engine malfunctions prior to the impact.
