Accident Notification
On April 28, 2016, an accident occurred involving an Embraer E-190 aircraft. The Direccion General de Aviacion Civil (DGAC) of Ecuador, the aviation authority for the state of occurrence, notified the National Transportation Safety Board (NTSB) of the United States about the accident.
Investigation Details
In response to the notification, the NTSB appointed a U.S. Accredited Representative to participate in the investigation. This appointment was made under the provisions of ICAO Annex 13, which governs international aviation accident investigations. The U.S. Accredited Representative assists the DGAC investigation as the State of Design and Manufacture of several subsystems of the airplane.
All investigative information will be released by the DGAC, as the lead investigating authority. The NTSB's role is limited to providing assistance and expertise related to the design and manufacture of aircraft components.
