What happened
On June 22, 2019, a skydiver performed a jump from an altitude of 4,000 meters near the Piotrków Trybunalski (EPPT) airfield. The individual, who held a PL. PJ (B) training class and had completed 231 jumps prior to the incident, utilized an Aerodyne Pilot 150 parachute manufactured in 2009.
The exit from the aircraft was executed without incident, and the freefall phase remained stable. At approximately 1,200 meters, the skydiver deployed the main parachute. The deployment process, including the deployment of lines and the canopy exiting the deployment bag, proceeded normally. However, during the landing phase, the skydiver performed an improper maneuver, resulting in a hard impact. Following the landing, the skydiver was unable to stand and reported significant back pain. Emergency medical services were dispatched to the scene, and the individual was transported to a hospital, where a spinal injury was confirmed.

The investigation examined the sequence of the jump, the deployment of the Aerodyne Pilot 150 parachute, and the landing conditions. The inquiry focused on the mechanics of the landing maneuver and the physical impact sustained by the skydiver.
