What happened

On 22 August 2013, a de Havilland DHC-3T Otter, registration C-FSGD, failed to arrive at its destination after departing from Scott Lake, Northwest Territories. The aircraft was operating a flight intended for an outpost camp within the region. When the operator reported the aircraft as overdue, the Rescue Coordination Centre dispatched a search aircraft to the coordinates supplied by the operator.

During the search, the wreckage was located approximately 7 nm southeast of Ivanhoe Lake. The impact resulted in the total destruction of the aircraft. The pilot, who was the only person on board, sustained fatal injuries.

The investigation

Following the discovery of the wreckage, investigators from the TSB and the RCMP dive team were deployed to the site to examine the scene. The investigation focused on the circumstances leading to the loss of the aircraft and the impact of any structural damage found at the crash site.
