What happened
On 26 July 2021, a Cessna A185F seaplane, registered as C-GYJX, was departing from the Tofino Harbour Water Aerodrome in British Columbia. The aircraft, operated by Atleo River Air Service Ltd., was transporting one pilot and four passengers toward the Hesquiaht First Nation community at Hot Springs Cove.
As the aircraft was performing its takeoff at approximately 11:38 Pacific Daylight Time, it briefly lifted off the water. However, the pilot subsequently lost control of the aircraft, causing it to cartwheel and land upside down in shallow water. Following the impact, the pilot and all four passengers managed to egress the aircraft and reach a nearby sandbar. The incident resulted in one serious injury and three minor injuries among the passengers, while the pilot sustained minor injuries. The aircraft sustained substantial damage during the accident.
The investigation
Investigators focused on the circumstances surrounding the loss of control during the takeoff roll. The investigation established that the aircraft encountered the wake of a boat during the critical takeoff phase. Additionally, it was noted that the 406 MHz emergency locator transmitter failed to send a signal following the occurrence.
