What happened
During the initial climb phase following takeoff, the right engine of the aircraft experienced a failure. In response to this mechanical issue, the crew declared an emergency and received clearance to return to the airport for an emergency landing. The flight proceeded through severe weather conditions characterized by thunderstorms, rain, and low ceilings.
While executing the final turn to align with the approach path, the aircraft suffered a loss of control. This resulted in a stall that led to the crash in the vicinity of the airfield. The impact resulted in one fatality and one serious injury. The aircraft was completely destroyed during the accident.
Findings
Investigations into the accident identified several contributing factors related to both mechanical maintenance and operational decisions. It was determined that the crew attempted to operate the aircraft despite known equipment deficiencies. Specifically, the right engine had been exhibiting rough operation prior to takeoff, a condition known to both the pilots and supervisors.
Technical findings highlighted issues with engine structure, specifically regarding valve assemblies, alongside inadequate inspections and maintenance services provided by the company. Operational errors also played a significant role, including poor judgment by the pilot-in-command, an improperly planned approach, and performing a go-around maneuver within heavy thunderstorm activity.
