What happened
During the initial takeoff phase at Kodiak Airport, a twin engine airplane encountered a critical loss of control immediately after leaving the ground. As the aircraft lifted off, it impacted a pile of gravel located near the runway. This impact led to an aerodynamic stall, causing the plane to crash. The accident resulted in two fatalities, as both occupants on board were killed in the event.
At the time of the departure, environmental conditions were unfavorable, with visibility significantly limited by the presence of fog across the airfield.
Findings
Investigations into the accident identified that the pilot did not maintain an adequate airspeed during the takeoff roll. Furthermore, the early stage of the takeoff sequence was conducted within foggy conditions at the southern end of the runway, which contributed to the circumstances leading to the failure to achieve sufficient takeoff speed.
