What happened
During a nighttime approach into the Greater Rockford Airport, the aircraft experienced a sudden, sharp left bank. This maneuver was followed by a rapid loss of altitude, leading the plane to impact the ground and catch fire. The impact resulted in the destruction of the airframe. Of the occupants on board, one fatality was recorded, while both members of the flight crew sustained serious injuries.
Findings
Investigations into the accident identified several environmental and operational factors. The aircraft encountered adverse weather conditions characterized by sleet and freezing rain. During the descent, the airframe accumulated moderate rime ice. The investigation concluded that the primary cause was a failure to maintain an appropriate flying speed, compounded by the improper use or failure to utilize necessary cockpit equipment.
