What happened
A cargo operation departing from Iola Airport experienced a catastrophic accident while en route to Olathes-Johnson County Airport. During the flight, the aircraft's communication systems, specifically the transmitter and receiver, ceased to function properly. As the pilot attempted an approach under deteriorating weather conditions, the aircraft entered a stall, resulting in a crash into a nearby field. The impact destroyed the aircraft, and the pilot, who was the only person on board, was killed.
Findings
The investigation identified several critical factors that led to the loss of control. The primary cause was an aerodynamic stall occurring after the flight was initiated despite adverse meteorological conditions. Contributing environmental and mechanical elements included:
- Severe icing conditions involving snow, sleet, and freezing rain
- Accumulation of ice on the wings and stabilizers
- Reduced visibility with a ceiling and horizontal sightlines of less than half a mile
- Total failure of radio communication equipment
- Inoperative navigation aids, specifically the destination NDB/ILS glideslope
- Accumulated airframe ice
