What happened
During an approach to Saint Augustine Airport, a passenger contacted air traffic control to report that the pilot had lost all control over the aircraft. The passenger indicated that the flight controls had become frozen. Shortly after this communication, the plane became uncontrollable and impacted the ground, resulting in a large fire at the crash site.
Both occupants of the aircraft were killed in the accident. The intensity of the post-crash fire was so severe that it completely destroyed the airframe, making it impossible for investigators to perform a detailed examination of the wreckage to identify specific mechanical failures.
Findings
Official investigations into the incident noted the presence of fog at the time of the accident. While the passenger's report suggested that the flight controls had frozen, the extensive damage caused by the fire prevented a definitive determination of the exact sequence of events or the specific reason for the loss of control.
