The impact resulted in the destruction of the aircraft and caused three injuries among the occupants.
Findings
The investigation determined that the crew did not adhere to approved procedures while conducting an IFR approach. Several environmental and operational factors contributed to the accident, most notably severe icing conditions involving sleet and freezing rain.
Additional contributing elements included:
- The presence of ice on both the airframe and the windshield
- Reduced visibility due to fog
- Improper in-flight decision-making by the crew
- Disregard for standard operating practices
- The deicer was turned off after visual references were established, causing the windshield to ice over
- Accumulation of ice rendered the flight unsustainable
