Engine failure and crash during initial climb

1 fatality • Fallon, United States of America • Takeoff (climb)

An aircraft experienced a right engine power loss during takeoff, leading to a crash approximately two miles from the departure runway.

What happened

During a flight intended to transport two political candidates, an aircraft experienced a critical loss of power in its right engine while climbing from runway 03. Following the engine failure, the aircraft lost altitude and crashed roughly two miles southeast of the airport. The aircraft had recently been brought back into active service following a five-year period of storage.

Findings

Investigations into the accident revealed that the pilot failed to execute the necessary emergency protocols following the engine failure. Specifically, the crew did not feather the propeller, failed to bank the aircraft toward the functioning engine, and did not close the cowl flaps on the non-operational engine.

Technical examination of the right engine identified several mechanical issues. There was evidence of overheating and erosion on the #1 and #4 pistons, which had developed holes along their top edges. The fuel system also showed significant degradation, including clogged fuel injectors, as well as contamination and corrosion within the fuel injector pump. Furthermore, the manifold valve was found to be partially obstructed and contaminated. The investigation also determined that the absolute pressure control for the turbocharger had been improperly adjusted.

Probable cause

The engine failure was caused by internal mechanical damage and fuel system contamination, compounded by the pilot's failure to follow established engine-out emergency procedures.
