What happened
During an approach to the South Naknek area, the aircraft encountered extremely challenging meteorological conditions, including fog, low cloud ceilings, and active icing. While on final approach, the plane impacted the ground a few miles from the airfield. The impact resulted in the total destruction of the aircraft and left the pilot, who was the only person on board, with serious injuries.
Findings
The investigation determined that the crash was caused by the aircraft striking a dirt bank during its approach after the flight had begun under unfavorable weather. Several environmental and operational factors contributed to the accident:
- Presence of ice on the windshield
- The pilot experienced spatial disorientation
- A failure to execute a go-around in a timely manner
- Visibility reduced to two miles or less
- Flight operations conducted below established minimums
- Severe weather including sleet, freezing rain, and icing
- Low cloud ceilings and fog
- Downwind wind conditions
