What happened
A chartered flight was operating a round-trip mission between Santa Maria, California, and Toledo, Ohio, to transport the California State Polytechnic College football team. During the return leg of the journey, departing from Toledo Express Airport, the aircraft experienced a catastrophic failure during its initial climb phase.
The aircraft struck a field located approximately 1.1 miles beyond the runway threshold, where it caught fire and was destroyed. The accident resulted in 22 fatalities, including both pilots and 20 passengers (which included 16 members of the football team), while 26 other occupants sustained injuries.
Findings
Investigations into the crash identified several critical factors. The aircraft was operating at a weight approximately 2,000 lb above its maximum certificated gross weight of 47,100 lb. Additionally, investigators noted unfavorable weather conditions and a partial loss of power in the left engine as contributing elements to the accident.
The primary reason for the crash was the loss of control during a premature lift-off.
