What happened
Approximately ten minutes after taking off from Newcastle Airport, the flight encountered extreme icing conditions. As a result of these environmental factors, ice began to accumulate on the leading edges of the aircraft. While maintaining a flat attitude at an altitude of 3,200 feet, the twin engine aircraft struck the ground and was consumed by fire. The impact resulted in the total destruction of the airframe. There were eight injuries reported among the occupants, all of whom sustained serious harm during the accident.
Findings
Investigations into the crash highlighted several critical failures regarding flight preparation and equipment. It was determined that the pilot initiated the flight without reviewing weather reports for the intended route, leaving him unaware of the impending icing conditions. Furthermore, a significant contributing factor was that the aircraft lacked necessary deice/anti-ice systems, which constituted a violation of established aviation regulations.
