What happened
During the takeoff phase from Athol-Silverwood, the aircraft experienced significant issues maintaining altitude. While performing its initial climb, the plane struggled to gain sufficient height and subsequently entered a stall. The aircraft impacted a prairie area situated beyond the runway threshold, resulting in a post-impact fire.
The accident resulted in three fatalities among the crew members. Additionally, all three passengers on board sustained serious injuries during the event.
Findings
Investigations into the accident identified an engine failure during the climb as the primary factor behind the loss of altitude. Several contributing elements were noted by officials:
- The flight crew did not attempt to abort the takeoff sequence once difficulties began.
- This was the first operational flight for the aircraft in roughly 11 months.
- The pilot's flying credentials were under a period of suspension at the time of the incident.
