On July 14, 2019, a Robinson R-22M helicopter, registration RA-1789G, arrived at the Shevlino landing site in the Solnechnogorsk urban district of Moscow Oblast. The pilot-in-command arrived at approximately 08:00 to begin pre-flight preparations. The pilot independently obtained all necessary aviation and meteorological information, and weather conditions were found to be compatible with the flight plan.
Prior to departure, the pilot-in-command verbally postponed the scheduled 10:06 departure to 11:00. Because there was no medical support available at the landing site, the pilot-in-command exercised the authority to authorize the flight under Federal Aviation Regulation 128. The aircraft's fuel gauges indicated approximately 73 liters of fuel in the tanks before takeoff.
The pilot performed an initial solo flight at 11:56, which included taxiing, hovering, and three landings. Following these maneuvers, the aircraft landed at the parking area at 12:11. No refueling was performed between the initial flight and the subsequent flight.
During the second flight, the aircraft entered a deep left bank accompanied by a rapid descent, leading to a collision with the ground. The accident resulted in two fatalities.
