A single-engine aircraft crashed during the initial climb phase following takeoff, resulting in the deaths of both occupants.

What happened

During takeoff, the aircraft began its rotation approximately 3,217 feet along the 3,746-foot runway. Witnesses reported that one or both engines were producing irregular running sounds during the takeoff roll. After lifting off, the aircraft reached an altitude of roughly 300 feet above ground level, at which point it entered a left bank and a nose-down pitch before striking the terrain. The accident resulted in two fatalities.

Findings

Investigation into the engine performance revealed several mechanical issues. The left engine required a new ignition harness, adjustments to the turbocharger density controller, and corrections to the magneto-to-engine timing to reach full rated RPM. The right engine also required several repairs, including the replacement of the magneto, ignition harness, and the engine-driven fuel pump. Additionally, a fuel injector nozzle in the #3 cylinder was found to be partially obstructed by contaminants. While the right engine magneto contact assemblies were functional, the capacitors had sustained heat damage.

The pilot's toxicology report indicated the presence of butalbital, a prescription barbiturate not authorized for use while operating an aircraft, as well as traces of acetaminophen and salicylate in the urine.
