Accident
An airplane departed from Dubois-Jefferson County Airport. Shortly after takeoff, during the initial climb, the pilot experienced engine trouble. The pilot attempted an emergency landing. However, the airplane went out of control and crashed near the airport. The accident resulted in the deaths of both occupants.
Investigation Findings
Investigators determined that foreign material affecting normal engine operations led to the loss of control. Several contributing factors were identified: ice in the fuel, fuel starvation, improper emergency procedures, improper operation of flight controls, complete engine failure, a forced landing off airport on land, and the presence of ice in the fuel flow divider and lines.
Probable Cause
The official probable cause was loss of control during initial climb due to foreign material affecting normal engine operations.
