Accident Sequence
A crew comprising a pilot and engineers was performing a local test flight. The aircraft departed from Fort Worth-Meacham Airport. During the initial climb after takeoff, both engines misfired. The pilot responded by shutting down the right engine and feathering its propeller. However, control of the aircraft was lost. The airplane crashed in the vicinity of the runway end. The impact destroyed the aircraft, and both occupants sustained fatal injuries.
Investigation Findings
Investigators determined that the probable cause of the accident was a failure of the powerplant ignition system, specifically due to spark plug failure. Several factors were identified as contributing to the accident. These included: inadequate maintenance and inspection by maintenance personnel; the pilot-in-command's failure to follow approved procedures; the pilot-in-command's failure to obtain or maintain flying speed; improper execution of emergency procedures; a partial loss of power affecting both engines; engine misfiring caused by fouled spark plugs; and the landing gear not being retracted.
