Summary
During the takeoff roll, witnesses observed the aircraft swerving left and right while the engine(s) ran intermittently or with missing power. The aircraft lifted off, climbed to approximately 50 feet above ground level, and continued across a waterway in a left turn with the landing gear extended. The aircraft then pitched up and rolled left wing down, impacting the water.
Event Description
The takeoff roll was characterized by lateral deviations and irregular engine performance. After becoming airborne, the airplane reached about 50 feet AGL and crossed a water body while turning left. The landing gear remained extended throughout. Subsequently, the nose pitched up and the aircraft rolled left wing down, resulting in collision with the water. The sole occupant, the pilot, was killed.
Examination Findings
Inspection of the engines revealed a cracked coil in the left magneto of the right engine. No other mechanical malfunctions or failures were identified.
Probable Cause
The official probable cause findings were:
- Occurrence #1: loss of engine power (takeoff roll). Findings: 1 engine; ignition system coil cracked.
- Occurrence #2: loss of control in flight (takeoff initial climb). Findings: improper emergency procedure; aborted takeoff not performed; improper in-flight planning/decision; operation with known deficiencies continued; airspeed (Vmc) not maintained.
- Occurrence #3: in flight collision with terrain/water (descent uncontrolled). Findings: terrain condition - water.
