About 15 minutes after departure, while climbing through FL170, the pilot reported a loss of both engines. The aircraft descended uncontrolled out of an overcast layer. At approximately 1,000 feet agl, the right aileron separated. The fuel boost pumps were found in the off position. Both occupants were killed.

Background

On the day of departure, the pilot received a 30-minute familiarization flight upon completion of a "superstar" conversion. Emphasis during this flight was directed to engine gauges, fuel flow and counter, and power settings. The familiarization flight was conducted at 5,500 feet.

Flight and Engine Failure

The pilot obtained a weather briefing and filed a flight plan. Approximately 15 minutes after departure, while climbing through Flight Level 170, the pilot radioed that both engines had been lost. The aircraft was subsequently observed descending uncontrolled out of an overcast layer.

Aileron Separation and Descent

At about 1,000 feet above ground level, the right aileron separated. The descent continued uncontrolled.

Post-Accident Findings

Examination revealed that the fuel boost pumps were found in the "off" position. The Airplane Flight Manual (AFM) states that the boost pumps should be on during climb above 10,000 feet.

Pilot Training History

The pilot had been enrolled in an Aerostar transition school the previous month. During that training, a two-hour combined demonstration and flight evaluation flight was conducted. The instructor pilot recommended further multi-engine training.

Occupants

Both occupants were killed.

Probable cause

Loss of engine power (total) – nonmechanical, due to pilot not following flight manuals (fuel starvation, improper fuel boost pump selector position) and lack of total experience in type; loss of control in flight due to inoperative engines, uncontrolled aircraft handling, spatial disorientation, and inadequate recurrent training; airframe/component/system failure/malfunction due to aileron overload and design stress limits exceeded leading to aileron separation; in-flight collision with terrain/water.
