What happened
On 2 April 2001, a Robinson R22, registration ZS-HUJ, was involved in an accident near Rietvlei in the Lydenburg district. The flight originated from Nelspruit, with the pilot traveling to Kudu Ranch to assist a friend in locating a wounded animal. Following a refueling stop at Nelspruit, the pilot and one passenger departed Kudu Ranch for a nearby farm, RietvLEI.
Prior to the final departure, the crew landed at Rietvlei to reduce the aircraft's weight by draining 30 liters of fuel, a measure taken due to the high ambient temperature and the altitude of approximately 4,200 feet. To facilitate a longer takeoff run, several shrubs had been cleared from the landing area. The pilot attempted a takeoff from the southern end of the clearing, positioning the aircraft to account for a northern wind and a downward slope toward the north. During the ascent, the pilot attempted a low-power takeoff; however, as the aircraft crossed the tree line, it failed to maintain altitude. The main rotor blades struck the trees, leading to a ground impact.
The investigation
Investigators examined the circumstances surrounding the takeoff, specifically focusing on the environmental conditions and the aircraft's performance. The investigation noted that the weather was clear with good visibility and temperatures of approximately 28°C. The inquiry also considered the physical modifications made to the takeoff area and the pilot's decision to utilize a limited-power technique. An eyewitness reported that the wind direction shifted during the takeoff sequence.
