What happened
On 29 June 2001, a BEECH 58, registered as ZS-MUY, was operating a private flight from Welkom to Douglas. The flight was being conducted by a pilot with a private license, accompanied by one passenger, for the purpose of attending a meeting.
Upon arrival at Douglas at approximately 1308Z, the aircraft performed a landing without the landing gear being extended. The pilot subsequently confirmed that the landing was a wheels-up event.
The investigation
Due to the nature of the event, no on-site investigation was conducted. The investigation relied on the statements provided by the pilot and a review of available documentation. The pilot reported that the landing gear was not lowered prior to touchdown and noted that the cockpit warning hooter was non-functional at the time of the incident. However, because there was no physical examination of the wreckage, the specific reason for the inoperative warning system could not be verified. Records indicated that the aircraft had been maintained according to requirements, and the pilot held both a valid license and a medical certificate.
