What happened
On 22 October 2001, a Robinson R22 helicopter, registration ZS-HAN, was involved in an incident during a solo training flight. The student pilot was returning to FAGM to land at a designated area. As the aircraft was performing a hover, the pilot applied power, which triggered a sudden and violent right-hand accelerating spin. This maneuver led to a hard landing on the airfield.
While the aircraft suffered significant damage to its engine frame and skids, as well as minor damage to the vertical stabilizer, the pilot was able to exit the helicopter without any injuries.
The investigation
Investigators reviewed the flight documentation and the aircraft's maintenance history. The investigation confirmed that the student pilot held a valid license and an unrestricted medical certificate. Furthermore, the aircraft was found to be properly maintained according to all required documentation. Meteorological reports indicated that weather conditions at the time of the incident were fine.
Findings
- The primary cause of the accident was the student pilot's failure to counteract the main rotor thrust during the landing phase.
- The aircraft was operating under a valid domestic air services license.
- The incident occurred during a routine training operation.
