What happened
On 9 June 2002, a Robinson R22 Beta helicopter, registration ZS-HYT, was performing a commercial game capturing operation in the Mkuze District. The aircraft had been flying for approximately one hour, completing several take-offs and landings to assist in moving wildlife into a boma. During the final flight segment, the pilot was maneuvering the aircraft to chase game into the enclosure.
As the animals were entering the boma, the pilot transitioned the helicopter into a hover and began an anti-clockwise turn. During this maneuver, the aircraft drifted rearwards, causing the tail rotor to strike the uppermost wire of a high-tension power line. The impact caused the helicopter to move rapidly backward and bank left by approximately 45 degrees. The nose of the aircraft subsequently dropped, sending the helicopter into an uncontrollable spin. The aircraft struck the ground in an inverted position, causing fatal injuries to the pilot and substantial damage to the airframe.
The investigation
SACAA AIID investigators examined the wreckage and interviewed witnesses at the Leeukop Game Farm, located roughly 30km east of Pongla. The investigation confirmed that the weather conditions were favorable, with good visibility and light winds. Maintenance records for ZS-HYT indicated that the aircraft had been maintained according to approved procedures.
Investigators also reviewed the pilot's credentials, noting that the pilot was licensed and qualified for the operation, though the flight was not being conducted under a valid Air Operating Certificate (AOC). The investigation focused on the sequence of the tail rotor's contact with the electrical infrastructure and the subsequent loss of control.
