What happened
On 22 May 2009, a student pilot was conducting his first solo consolidation flight at Grand Central Aerodrome (FAGC). The flight, operated by Air Training Services, followed an earlier training session with an instructor that had covered various maneuvers and emergency procedures without incident. During the initial takeoff phase, while attempting to transition into a hover, the Robinson R22 Beta, registration ZS-RHC, experienced a sudden loss of stability. The aircraft's left skid made contact with the ground, triggering a dynamic rollover. Because the event occurred at a very low altitude, the pilot was unable to implement recovery maneuvers before the helicopter rolled onto its right side.
The investigation
An investigation by the SACAA AIID examined the circumstances surrounding the accident, which occurred at approximately 0833Z under favorable weather conditions (CAVOK). Investigators reviewed the maintenance records of the aircraft, finding that the helicopter was properly maintained with a valid certificate of airworthiness. The investigation also confirmed that the student pilot held a valid license and medical certificate. Physical examination of the wreckage revealed that the main rotor struck the ground and the windscreen was shattered, resulting in substantial damage to the main rotor and skids. No mechanical malfunctions or environmental factors were identified as contributing to the event.
