Single-engine power loss during descent leads to fatal aircraft accident

2 fatalities • Winslow, United States of America • Flight

A private flight ended in a crash near its destination airport after the aircraft experienced an undetermined loss of power in one engine during the descent phase.

What happened

During a personal flight, the pilot was performing a descent toward the destination airport. Surveillance data via ADS-B indicated that the aircraft performed several turns while descending from an altitude of 6,000 feet to approximately 4,95 and 4,950 feet mean sea level. At the point where tracking data ceased, the aircraft was positioned only 80 feet above the ground. Eyewitnesses at the scene observed the aircraft flying at a low altitude, executing a turn, and subsequently veering toward the earth.

The aircraft eventually came to a stop roughly 4 miles east of the intended airport, approximately 70 feet from the final recorded position. A fire broke out following the impact. There were no survivors reported in the wreckage.

Findings

An inspection of the airframe and engines revealed no mechanical issues that would have prevented the aircraft from operating normally. While the left engine appeared to be producing power at the time of the crash, the right engine showed evidence of low or no rotation. Specifically, the right propeller shaft exhibited localized contact marks, and the propeller blades showed chordwise scoring with a twisted, low-pitch orientation.

No debris or metal spray was found in the turbine section, and the fuel system showed no irregularities. Evidence from the autostart computer's annunciator panel indicated that the right engine's SRL-OFF light was illuminated during the accident. Based on the lack of rotation in the right engine and the active warning light, investigators concluded that the right engine lost power for reasons that remain undetermined.

Probable cause

The right engine experienced an undetermined loss of power during the descent phase of flight.
