Engine explosion during takeoff at Ukrainka Air Base

2 fatalities • Ukrainka AFB, Russia • Takeoff (climb)

A training flight involving a dual-registered aircraft experienced an engine explosion during its takeoff roll, resulting in a runway excursion and multiple casualties.

What happened

During a scheduled training mission departing from Ukrainka Air Base, an aircraft with registration RF-94181 (also identified as 05 red) experienced a critical mechanical failure. As the aircraft was performing its takeoff roll, one of the two left-side engines suffered an explosion and subsequent fire.

Following the engine failure, the aircraft became uncontrollable. The pilot attempted to execute emergency braking procedures, but the plane veered off the runway and performed a 180-degree turn before coming to a complete stop. A significant fire broke out, resulting in the partial destruction of the aircraft type.

Findings

There were nine crew members on board at the time of the incident. The accident resulted in nine total injuries, with the pilot sustaining serious wounds. While the initial impact and fire caused immediate injuries to all personnel, two of the survivors later succumbed to their injuries several hours after the event.

Probable cause
