The National Transportation Safety Board (NTSB) has issued Recurrent Safety Recommendation A-12MA020, designated as A-13-003, addressed directly to the Federal Aviation Administration. This recommendation urges the agency to require that all personnel performing maintenance or inspections under 14 Code of Federal Regulations Parts 121, 135, 145, and 91 Subpart K receive comprehensive initial and recurrent training focused on human factors affecting maintenance. The mandated curriculum must include a detailed review of the causes of human error, with specific attention to fatigue, its effects on performance, and the actions individuals can take to prevent the development of fatigue during their duties.
