The National Transportation Safety Board (NTSB) has issued a safety recommendation, designated A-14-087, addressed to UPS Airlines. The directive, associated with NTSB number DCA13MA133, urges the carrier to work directly with the Independent Pilots Association. This collaboration is intended to conduct an independent review of the company's fatigue event reporting system. The primary objective of this review is to determine how effective the current program is as a nonpunitive mechanism for identifying and addressing reported fatigue issues among flight crew members.
