The National Transportation Safety Board has issued a safety recommendation to Pratt and Whitney Canada regarding the reversing lever guide pin.

The National Transportation Safety Board (NTSB) has issued safety recommendation ERA15FA129, designated as A-16-054. This directive is addressed to Pratt and Whitney Canada. The board urges the manufacturer to complete the development and implementation of a redesigned guide pin assembly for PT6A engines. The primary objective of this redesign is to prevent the possibility of the reversing lever, also known as the beta arm, and the guide pin being incorrectly installed. The NSB states that these actions should be taken as soon as practicable to mitigate potential safety risks associated with improper assembly of these critical engine components.
