- 2025-11 B767-300 Captain was informed by gate agent that aircraft cleaners deployed escape slide. Maintenance was called; flight was cancelled after determining there was significant damage to multiple sections of the aircraft.
- 2025-08 Air Carrier Captain reported during taxi for takeoff hearing multiple loud bang sounds resulting in the flight crew not accepting the takeoff clearance and returning to the ramp where it was discovered that a cargo bin was not secured.
- 2025-06 B767 Technician discovered the escape slide dropped during an overnight maintenance check; repair of window shade; and removal of emergency door.
