FLC OF LGT TOOK OFF WITH R GND SPOILERS EXTENDED.

Narrative

THIS IS A FOLLOW UP FOR YOUR INFO; RPT FILED ON THIS INCIDENT 12/X/91. LGT CAME OUT OF 'C' CHK LOCKING PIN NOT REMOVED FROM R GND SPOILERS. GND SPOILERS LOCKED IN FULL EXTENDED POS (SHOULD HAVE HAD RED FLAG ATTACHED TO PIN; NONE ATTACHED). NOT DETECTED ON PREFLT AS COMPANY PROC IS TO DEPLOY ALL SPOILERS PRIOR TO OUTSIDE WALK AROUND; SO ALL SPOILERS WERE UP DURING FLT ENGINEER CHK; WHICH IS NORMAL. THERE IS NO REQUIREMENT TO CHK SPOILER RETRACTION ONCE SPD BRAKE HANDLE RETURNED TO STOWED POS. SO! WE TOOK OFF WITH SPOILER UP. IT CAUSED ROLL AND YAW PLUS CAUSED #3 ENG TO VIOLENTLY COMPRESSOR STALL CONTINUOUSLY UNTIL SHUT DOWN. ROLL PROBLEM QUITE SEVERE. 5 UNITS RUDDER 50 DEG L AILERON -- TO FLY STRAIGHT -- VERY LITTLE ABILITY TO TURN L. (THRUST FROM #1 PLUS SPOILER). LANDED UNEVENTFULLY. FORTUNATELY I AM AN LGT CHK CAPT AND WE EXPERIMENTED AROUND WITH THIS STUFF IN THE SIMULATOR ALL THE TIME; SO IT WASN'T A BIG DEAL; BUT COUPLED WITH BAD WX OR MEXICO CITY; ETC. IT COULD HAVE BEEN A DISASTER. SUPPLEMENTAL INFO FROM ACN 195898. AFTER TKOF (CAPT FLYING) ACFT FLT CTLS HAD A PROBLEM AND #3 ENG STARTED COMPRESSOR STALLING. A PRECAUTIONARY SHUTDOWN WAS ACCOMPLISHED AND FUEL WAS DUMPED. FLT CTLS STILL HAD A MAJOR PROBLEM. AN EMER WAS DECLARED AND FIRE TRUCKS WERE STANDING BY FOR US. WE FOUND OUR INBOARD R GND SPOILERS WERE FULLY DEPLOYED. AT THIS TIME MAINT THINKS THAT A LOCK OUT WAS FORGOTTEN BY MAINT AFTER A HVY 'C' CHK; AND THE DISRUPTED AIR AFTER TKOF FLAMED-OUT OUR #3 ENG; AND CAUSED THE OBVIOUS FLT CTL PROBLEMS.
