NMAC.

Narrative

THE OCCURRENCE HAPPENED ON JAN/XX/92 AT APPROX XA30 AM LCL TIME. IT INVOLVED 2 ACFT; 1 AN SMA AND THE OTHER AN SMT. I AM A FLT INSTRUCTOR AND WAS CONDUCTING FLT TRAINING FROM THE R SEAT IN THE SMA. WE HAD BEEN DOING TOUCH AND GO LNDGS AND USING RWY 18 AT MAYFIELD; KY. AT THE TIME OF THE INCIDENT WE WERE ON DOWNWIND; AND ACCORDING TO WITNESSES THE SMT HAD MADE A MIDFIELD ENTRY OVER THE RWY TO THE DOWNWIND OF 18. WE HAD BEEN CONSTANTLY RPTING ON DOWNWIND; BASE; AND FINAL LEGS. THE RADIO SPEAKER IN OUR SMA ISN'T THE BEST AND A LOT OF THE MESSAGES TEND TO COME OUT A LITTLE GARBLED; SO I DON'T RECALL HEARING THE SMT RPT ENTERING THE PATTERN. FOR THAT MATTER I DON'T THINK HE REQUESTED AN ADVISORY ON THE UNICOM AND WITNESS IN THE FBO SAID HE DID NOT CALL FOR AN ADVISORY. A WITNESS ON THE GND SAID THAT THE SMT BASICALLY WAS FLYING OVER US AFTER HE MADE A SHORT TURN TO BASE; THEN WE WERE BOTH ON FINAL AND DSNDING TO THE RWY WITH THE SMT ABOVE US AND GETTING CLOSER. THE WITNESS SAID THAT THE SMT CAME WITHIN A FEW FT BEFORE HE SAW US AND MADE A L HAND GAR TURN. WE HAD NEVER SEEN THE PLANE AND DIDN'T KNOW WHAT HAD TRANSPIRED UNTIL WE LANDED AT THE END OF OUR LESSON. AFTER THE NEAR MISS WE CONTINUED DOING TOUCH AND GOES. THE FIRST TIME I HAD SPOTTED THE SMT WE WERE ON DOWNWIND AND A LITTLE PAST ABEAM THE NUMBERS WHEN HE WAS XING OVER THE RWY AGAIN AT MIDFIELD FOR AN ENTRY TO DOWNWIND. I THEN NOTICED THAT HE WAS MAKING A RPT FOR EVERYTHING HE DID MORE THAN NORMAL. FOR EXAMPLE HE RPTED ON FINAL; SHORT FINAL; AND BACK TAXIING ON THE RWY. DURING HIS LNDG WE CONTINUED TOUCH AND GOES. I KNOW THE PLT OF THE SMT AND CONSIDER HIM TO BE A VERY COMPETENT PLT PLUS HE IS AN EX-COMMUTER PLT. ALSO; ON HIS LNDG HE WAS BEHIND US IN THE PATTERN AND AS WE DID OUR TOUCH AND GO I NOTICED A DOG OFF OF THE RWY THAT APPEARED TO BE NO FACTOR TO THE SMT BUT I DECIDED TO ALERT HIM ANYWAY. WHEN I DID; I NOTICED THAT THERE WASN'T A RESPONSE FROM HIM; AND I THEN NOTICED THAT THE MICROPHONE JACK WAS IN ITS JACK BUT NOT ALL THE WAY TO MAKE A GOOD CONNECTION. WHEN WE FINALLY LANDED THE SMT WAS TAXIING BACK OUT AFTER PICKING UP HIS PAX; THEN I WAS NOTIFIED OF THE INCIDENT FROM ONE OF OUR EMPLOYEES AFTER HE WAS NOTIFIED BY A CALL FROM THE ABOVE WITNESS WHO WATCHED FROM THE GND. I BELIEVE THIS SITUATION OCCURRED DUE TO AN UNSAFE ENTRY TO THE PATTERN BY A BIGGER TWIN AT AN ARPT WHERE KNOWN STUDENT FLT TRAINING TAKES PLACE REGULARLY. ALSO; MISCOM BY BOTH ACFT MAY HAVE CONTRIBUTED.
