2023-06 · NASA ASRS report 2010891
Air carrier Captain reported a Hazmat loading location discrepancy between the Auto DG load report and the FOM. DG staff convinced the Captain that the DG load report was acceptable and flight departed safely.
First Officer approached me after his Dangerous Good (DG) inspection and said there was a 2.2 non flammable gas and dry ice in the accessible can but it was listed as an inaccessible. He told me he asked the DG staff why it wasn't listed as accessible and was told that she had worked DG for 15 years and that it was always listed this way. I reviewed the FOM and told him 2.2 needed to be in the accessible can and that it should be listed and loaded as accessible. The paperwork was then shown to me and it was pointed out that it was listed as 2.2 W/O CAO so it did not need to be listed as accessible even though that was where it was loaded; again she insisted this was normal. Ultimately we were convinced it was okay so we accepted the paperwork and pushed. Enroute to ZZZ we continued a discussion about the paperwork and upon further review it seems the DG in question was listed in 1L and 2L positions along with some dry ice. On reflection this paperwork error; if there was one; should have been further investigated by myself prior to push. Auto DG report possibly did not reflect actual load. Ensure accuracy of auto DG to load.
