B737-800 pilot reported a near miss with a ground vehicle while taxiing into the gate under ramp agent guidance. The Captain applied the brakes to prevent a collision as the van crossed the path of the aircraft; no injuries were reported.
Event Summary: Flight ABCD Ground Incident at ZZZ; Gate XXWho: The event involved the flight crew of Flight ABCD (ZZZ1 to ZZZ); two wing walkers; and the driver of a van at ZZZ.What: Day 0; at approximately XA:36; after landing and taxiing to Gate XX; Flight ABCD was under the guidance of wing walkers who had their wands raised; signaling the aircraft to proceed. A van crossed the path of the moving aircraft from left to right; disregarding the wing walkers' signals. The Captain of Flight ABCD applied the brakes to avoid a collision; narrowly missing the van; which was moving too quickly to stop and made no attempt to do so.When: The incident occurred on Day 0; during the parking phase at Gate XX; shortly after the aircraft completed taxiing from the runway.Where: The incident took place at ZZZ; at Gate XX; in the ramp area near the terminal. Why: The incident was caused by the van driver's failure to adhere to standard Ground Operation protocols; specifically ignoring the wing walkers' signals and crossing the active path of a moving aircraft. This suggests a lapse in situational awareness or disregard for safety procedures by the van driver.Threats:• Unauthorized Vehicle Movement: The van's movement across the active ramp area without clearance posed a significant collision risk.• High-Speed Operation: The van was traveling at a speed that prevented it from stopping in time; increasing the severity of the threat.• Potential Communication Breakdown: The van driver's disregard for the wing walkers' signals indicates a possible failure in communication or adherence to ground control protocols.Errors:• Van Driver Error: The van driver failed to yield to the moving aircraft and ignored the wing walkers' raised wands; violating standard ramp safety procedures.• Potential Wing Walker Error: While the wing walkers had wands raised; it is unclear if they took additional actions (e.g.; attempting to signal the van directly) to mitigate the van's approach; which could indicate a missed opportunity to prevent the incident.• Ground Operations Oversight: The incident suggests a potential lapse in ground crew coordination or monitoring; allowing the van to enter the active aircraft movement area.Outcome: The Captain's quick reaction to apply the brakes prevented a collision; and no injuries or damage were reported. However; the incident highlights significant safety concerns regarding ground vehicle operations and adherence to ramp protocols.Cause: Recommendations: A committee should investigate the van driver's training and adherence to safety protocols; review ground operations procedures at ZZZ; and consider enhanced communication or barriers to prevent unauthorized vehicle movements in active ramp areas.
