B747-800 Captain reported during a maintenance log review found a series of recorded and undocumented exceedances that may have resulted in potential aircraft damage. The captain refused the aircraft due to maintenance inspections not completed.

Narrative

Unsafe Conditions reasonably believed that such continued flight or action may have resulted in substantial and imminent risk of harm to the Crew Member or his equipment.Cause: On or about Day 0 a maintenance log review found- A series of recorded and undocumented exceedance's that may have resulted in potential aircraft damage. - Maintenance failed to perform Transit and Daily checked in accordance with Company Air procedures (see work cards for high Energy Stop review")- Maintenance Controller failed to act on AHM (Aircraft Health Management) notifications from AHM for 'Brakes exceed operational Limits' there by requiring local maintenance to perform 'High Energy Stop inspection.'- Potential conspiracy to conceal aircraft maintenance defects- Whereby [The] Maintenance Controller and Local maintenance falsified official maintenance records by:> Maintenance Controller ignoring AHM notifications> Local maintenance Deleting present leg faults (deleting official maintenance records)> Local Maintenance falsely issued airworthiness release into official aircraft logbook> and other potential violations. - Aircraft had flown approx 5 days beyond a required "High Energy Stop" inspection without maintenance intervention.- Action taken: Aircraft was found to be out of inspection and deemed "unsafe for further flight by the captain after reviewing maintenance records".- Exceedances were confirmed after review with local maintenance along with a call to dispatch and Maintenance Control confirming the inspection was required after Maintenance Control reviewed the "AHM" data stored on their end.Call with dispatch ended with all three parties agreeing to file reports. Suggestions: Mandate Fleet wide audit of SMS (Safety Management Systems)Mandate Fleet wide audit of AHM data relating to Brake overheatsMandate retraining of all crews and the Training department to recognize 'BRK TEMP' EICAS messages as 'HIGH ENERGY STOP' eventsRequire all crews to document 'BRK TEMP' Messages in the Aircraft paper logbookProvide training to comply with required reports of the FOM X.X.X and required reporting of 'High Energy Stops.'"
