Synopsis

CAPT OF AN MDT FAILED TO FOLLOW THE TRACK AND SPD CLRNC INSTRUCTIONS DURING DSCNT; FAILED TO TURN OFF RWY AFTER LNDG ON ASSIGNED TXWY AND FAILED TO NOTE A BELOW MINIMUM READING OF ACFT HYD QUANTITY; ALL DURING A FAA ENRTE CHK.

Narrative

WHILE FLYING FROM ALB TO LGA NUMEROUS MISTAKES OCCURRED DURING OUR RTE/FLT CHK BY THE FAA. THE PF FLEW PAST TRESA INTXN INSTEAD OF FLYING TOWARD CARMEL VOR. (THIS SAME INCIDENT HAPPENED LATER IN THE DAY). CAPT INSISTED ON USING WRONG ALTIMETER SETTING BEFORE LNDG. CAPT DID NOT TURN TO 270 DEGS AFTER PASSING LGA VOR AS INSTRUCTED. CAPT BEGAN DSCNT AT 30 KTS FASTER THAN HE WAS INSTRUCTED TO DO. LGA EXPRESSWAY VISUAL WAS NOT BRIEFED BEFORE APCH AND CAPT FLEW APPROX 3/4 MI OFF COURSE NEAR JFK'S AIRSPACE. CAPT HAD OUTDATED NAV CHART. TWR TOLD HIM TO USE HIGH SPD TURNOFF JULIET; HE TURNED ONTO REVERSE LIMA. HYD QUANTITY WAS BELOW MINIMUM AS CAPT RESPONDED TO CHKED ON CHKLIST RESPONSE. I REALIZE THAT I'M AS GUILTY AS THE CAPT ON A LOT OF THE INCIDENTS. I CAUGHT A LOT OF HIS MISTAKES DURING THE DAY AND HAD TO KEEP MORE ALERT THAN NORMAL. THE CAPT NEEDS TO LISTEN MORE TO THE RADIO AS WELL AS WHAT HIS CREW IS TRYING TO TELL HIM. HE TALKS CONSTANTLY ABOUT HIS SCHEDULE AND HOME LIFE WHILE MISSING CRITICAL ELEMENTS OF FLYING. I MUST BE MORE ASSERTIVE AT THE RISK OF BEING CALLED A JERK. I KNEW THE CAPT HAS FAILED OTHER CHK RIDES AND I FEEL HE IS NOT SAFE TO FLY THIS ACFT. I'M ALMOST SURE THAT THE SAFETY INSPECTOR WILL RPT HIS FINDINGS AND FUTURE ACTION WILL BE TAKEN FOR THE SAFETY OF OUR PAX. I WILL SPEAK TO THE UNION'S SAFETY COMMITTEE; HOPING TO INFORM THE CAPT OF HIS FAULTS.
