Synopsis

FLC OF AN MLG FAILED TO HAVE AN MEL'ED ITEM NOTED ON THE COMPANY DISPATCH FLT RELEASE WHICH IS CONTRARY TO COMPANY PROCS AND THE FARS.

Narrative

WE WERE PARKED AT THE GATE BTWN FLTS. THE MAP MODE OF MY EHSI HAD FAILED. VOR MODE; BOTH FULL AND EXPANDED WORKED NORMALLY. ALL CAPT'S INSTS FUNCTIONED NORMALLY. MAINT WAS NOTIFIED AND DECIDED TO MEL THE MAP MODE OF FO'S EHSI. FLC AGREED AS THIS WAS NO COMPROMISE TO SAFETY. I WOULD USE VOR MODE; WHICH PRESENTS ONLY VOR INFO ON MY HSI. TECHNICIAN NOTED MEL PROPERLY IN MAINT LOG AND PLACED MEL STICKER NEAR EHSI AS REQUIRED. WE DEPARTED FOR OUR NEXT LEG OF THE FLT BUT FORGOT TO HAVE OUR DISPATCH RELEASE AMENDED TO REFLECT OPS WITH AN MEL'ED ITEM; AS REQUIRED BY COMPANY RULES AND FARS. THIS MAY NOT SOUND LIKE A BIG DEAL TO A NON PART 121 PERSON; BUT IT COULD BE A BIG DEAL TO THE FAA. CONTRIBUTING FACTORS I BELIEVE WERE CREW FATIGUE. WE HAD ARRIVED FROM THE LAST LEG OF THE TRIP ABOUT 2 HRS LATE DUE TO WX. PART OF THE DELAY WAS A GND DELAY OF ABOUT AN HR AND ALSO 2 NON PRECISION NDB MISSED APCHS IN NON RADAR ENVIRONMENT AND FINALLY LNDG AFTER A THIRD NDB APCH. HVY WORKLOAD. WE PLACED PRESSURE ON OURSELVES BY TRYING TO TURN THE PLANE QUICKLY FOR OUR NEXT DEP SINCE WE WERE RUNNING SO LATE. HASTY PREFLT ACTIONS INTERFERE WITH CONCENTRATION WHICH I BELIEVE WAS A FACTOR IN THIS FORGETFULNESS. ALSO; MAINT NORMALLY COORDINATES WITH DISPATCH AND THEY INITIATE ACTION TO AMEND OR ISSUE A NEW DISPATCH RELEASE. THE FLC IS NORMALLY NOTIFIED WITHOUT US HAVING TO ASK. IN THE FUTURE; I WON'T DEPEND ON OTHERS TO MAKE SURE OUR PAPER WORK IS IN ORDER.
