Synopsis

ANTI-ICE PANEL INSTALLED UPSIDE DOWN.

Narrative

ON DEC/AA/95 AT APPROX XX00 HRS; MYSELF AND TWO OTHER MECHS WERE TROUBLESHOOTING THE BACKGROUND LIGHTS ON A FK10 ACFT'S STANDBY COMPASS. WE HAD REMOVED THE PANEL WHICH CONTAINS THE ANTI-ICE SWITCHES. AFTER DETERMINING WE HAD A BROKEN WIRE MY CO-WORKER INSTALLED THE ANTI-ICE PANEL BACK UP ON THE OVERHEAD CONSOLE. I PLACED THE STANDBY LIGHTING ON THE COMPASS ON MEL. SIGNED OFF THE TRANSIT CHK AND AIRWORTHY RELEASE. THE ACFT RETURNED THE NEXT NIGHT AND I REVIEWED THE LOGBOOK AND NOTICED THAT ON THE 5TH FLT AFTER MY ENTRIES; THE ANTI-ICE PANEL WAS WRITTEN UP FOR BEING UPSIDE DOWN. ANOTHER MECH REINSTALLED PROPERLY OPS CHK SATISFACTORY; NO DELAYS OR PROBS RPTED BY ANY OTHER FLC UP TO THE 5TH FLT. ACTUAL LOGBOOK WRITE-UP 'ANTI-ICE PANEL INSTALLED UPSIDE DOWN.' I MYSELF DID NOT REMOVE THE PANEL OR INSTALL THE PANEL. AT NO TIME WERE THE CANNON PLUGS DISCONNECTED. I FEEL WE ALL GOT IN A HURRY TO GET ACFT READY FOR DEP. I DID NOT CHK THE WORK OF MY CO-WORKER; ALSO AN A&P MECH; BEFORE I SIGNED AIRWORTHY RELEASE. TO PREVENT THIS FROM HAPPENING AGAIN; WE SHOULD OF NEVER GOT RUSHED AND THE PANELS COULD HAVE INDEXED MARKS ON THEM.
