HAZARDOUS CARGO PAPERWORK NOT COMPLETE AND POST FLT EXAMINATION FOUND IMPROPER WTS. THE HAZMAT WAS OVER MAX WT ACCORDING TO ACR OPS MANUAL. LRG.

Narrative

10 MINS PRIOR TO DEP; CONTACT AGENT BROUGHT ME A HAZARDOUS CARGO FORM. OUR AIRLINE'S FORM REQUIRES 3 SIGNATURES. THIS FORM GIVEN TO ME ONLY HAD 1 SIGNATURE; WITH THE OTHER 2 SIGNATURE BLOCKS NOT SIGNED. I CHALLENGED THE AGENT AND TOLD HIM THE FORM WAS INCOMPLETE AND THAT THE CARGO WOULD NOT BE CARRIED UNTIL THE PAPERWORK WAS COMPLETE. HE COMPLAINED ABOUT HAVING TO GO TO THE CARGO AREA WHICH WAS SOME DISTANCE AWAY. BUT SHORTLY AFTER LEAVING THE COCKPIT; HE RETURNED WITH 'SIGNATURES' IN THE REQUIRED BLOCKS. ENRTE FROM TPE TO KSS I BEGAN THINKING ABOUT THIS INCIDENT. I REVIEWED THE FORM AND STUDIED THE CARGO WHICH WAS 2 BOXES OF ENAMEL; SPECIFIED IN LITER QTYS; NO WT GIVEN. UPON ARR IN KSS; I HAD THE HAZARDOUS CARGO OFF LOADED. THE BOXES WERE MARKED 51 KG AND 33 KG RESPECTIVELY. OUR COMPANY OPS MANUAL SPECIFICALLY STATES THAT THE MAX WT OF THIS CLASS OF CARGO CARRIED IN ANY ONE BIN IS 25 KG. HAD I BEEN GIVEN MORE TIME BEFORE DEP TO ANALYZE THE CARGO AND CHK IT AGAINST THE OPS MANUAL; I MIGHT HAVE CAUGHT THE ERROR IN TPE. THE ADDED PROB OF THE MISSING SIGNATURES MINS BEFORE SCHEDULED DEP FURTHER CONFUSED THE SIT. TRAINING AND STRICT ADHERENCE TO ESTABLISHED PROCS WOULD PRECLUDE FURTHER OCCURRENCES OF THIS TYPE.
